Health Home Care Services

A Health Home is not a building or place. It is a team of health care providers and other professionals who work together to make sure an adult or child's health care needs are being met. The team is led by one of The Guidance Center of Westchester's care coordinators. Care coordination services are personalized, with a focus on achieving positive results for the participant. We offer care coordination services for adults and children.
