Assertive Community Treatment (ACT)

ACT is a mobile team-based approach to mental health care. Together, this team of multi-disciplinary mental health professionals works to develop and implement community-based services for people whose needs have not been met by the traditional office-based approach to mental health.

the ACT team

The ACT team consists of a team leader/program director, psychiatrist, employment/vocational specialist, substance use counselor, registered nurse, housing specialist, peer specialist, and family specialist.

ELIGIBILITY

ACT services are available to Medicaid-enrolled adults (18+) who have serious and persistent mental illness and a history of hospitalization. All participants are referred through Westchester County Single Point of Access (SPOA).
